Sports at CLPMS

Registration (complete the three steps listed)

Step One: Athletic Registration

Registration begins for all athletic seasons July 1st. You no longer need to wait for each season to register. Registration will close at the start of each sports season.

Step Two: After registration you will also need to complete SchoolPay ($140 per sport).

Step Three: Physicals are good for 1 calendar year. Please have this Sports Physical form signed by a Physician. Form Link

Free and Reduced Fee Waiver linkThis is a 3 step process that takes time. Please plan accordingly. 

PSD Health Center and Sports Physicals

The Health and Wellness Center at Centennial High School offers sports physicals/well child exams June - August. The Center can bill insurance and no child is ever turned away for inability to pay. For more information visit https://hwcenters.org/ or call their office at 970-488-4900.

Winter Coaches:

Winter practice times 2:35 - 4:35     Monday through Friday

* Changes in any practice times will be communicated by coaches
